Incident Data Form

The Incident Data Form allows you to document and send details of an incident to a supervisor and administrator for review and approval prior to submitting the form.


  1. Go to: Avatar PM > System Maintenance > Incident Data Form
  2. To complete a new Incident Data Form, select Add and then complete the required fields + any other fields that are appropriate for the incident.

Step-by-step order for Incident Data Form Workflow:

  1. Incident Occurs.
  2. Staff member enters the incident data into the Incident Data Form.
  3. The system sends a To-Do item to a supervisor so that they can review the incident.
  4. Supervisor reviews section one of the Incident Data Form.
    1. If approved, the system sends a To-Do item to an Admin to review the incident.
    2. If denied, the system sends a To-Do item to the original author to make changes accordingly.
  5. Admin will review both sections of the Incident Data Form.
    1. If approved, End of process.
    2. If denied, the system sends a To-Do item to the author of the section that was denied, so that they can make changes accordingly.

Note: This cycle may continue as many times as necessary before the Incident Data Form is efficient and complete.
