Foster Home Licensure for Care

Record data regarding the foster home’s ability to meet certain levels of care for the foster child.

Go to: Avatar MSO > Provider Management > Foster Home Licensure for Care
In the Licensed For Level Of Care field, select the authorized care level for which the Foster Home is licensed.
In the Effective Date and Expiration Date fields, enter the date range for the time period of licensure.
In the Comments field, enter any notes as necessary.
Click Submit.
