Foster Home Initial Registration

Record and track Foster Care Program home applications.

  • Within myAvatar, the foster home is considered a provider.

  1. Go to: Avatar MSO > Provider Management > Foster Home Initial Registration
  2. In the Select Provider menu, search for and select an existing provider to register as a foster home.
    If the provider is not found, click New Provider to enter a new provider as a foster home applicant.
  3. In the Application Date enter the date of the application to register as a foster home. This field is used to track the initial date of application for involvement with the Foster Care program. This date is display-only for existing providers.
  4. In the Name field, enter the name of the foster home. Required format is: LAST, FIRST.
  5. In the Primary Home Contact, enter the name of the primary contact person for this home. 
    This is a preliminary step for gathering data prior to full registration, and does not actually register the person as a Foster Home Member.
  6. In the Preferred Contact Method field, select the contact person's preferred way of being reached.
  7. In the Home Telephone field, enter the telephone number for the home.
  8. In the 'Home Address' fields, enter the address information for the home.
    The County can be added, as needed.
  9. In the Comments field, enter any relevant notes to accompany the application.
  10. Click Submit.
