Foster Home Member Associations
Assign foster home members to a registered foster home. Foster home members can be assigned to multiple foster homes at once.
Prerequisites:
- The foster home's application must be filed in the Foster Home Initial Registration form.
- Foster home members must be registered in the Foster Home Members form.
- In the Select Provider menu, search for and select an existing foster home member to assign to a foster home.
- The Display field displays a report of data regarding the foster home.
- Go to: Avatar MSO > Provider Management > Foster Home Members Associations
- The Foster Home Member Associations table lists all of the existing associations on file for this foster home. Click Add New Item to enter a new association.
- In the Foster Home Member field, select the registered foster home member to assign to this home.
- In the Foster Home Member Role field, select this person's role in relation to the foster child.
- If Other, enter a description of the undefined role in the Other field.
- In the Primary Home Contact field, select whether or not this member is the primary contact person for the foster home. It is assumed that the Primary Home Contact is the person to whom the check will be issued.
- In the Status field, select the status of the foster home member for the defined time frame.
- Active - Member is active and can be in the home with the child.
- Expired - Member was active but due to requirements, the status has expired.
- Pending - Member is in the process of completing requirements.
- Retracted - Member was active, but due to certain circumstances, the Foster Care program retracted the active status.
- In the Effective Date, enter the date on which the indicated status applied.
- In the Expiration Date, enter the date on which the indicated status terminated, if applicable.
