Reactivate an assessment in Flowsheet

You can reactive an assessment for a patient that was voided in error. 

Note: Only the person who authored the original assessment can reactivate the assessment. 

Prerequisites:

• Signed assessment is voided

1. Sign in to myAvatar.
2. Select a client/patient. 
3. Select an episode.
4. Select the Flowsheet console.
5. Select Include Voided Results.
6. Select the dots menu for the voided Flowsheet assessment. 
7. Select Reactivate. 
8. Enter a reactivate reason. 
9. Select Submit.
