Redefine Wound

Wounds can be redefined in myAvatar Flowsheet. Wounds can only be redefined one time and only by clinicians with access to redefine wounds within Flowsheet Configuration.  

Use Case: A nurse admits a patient and adds a new wound as a skin tear with an initial assessment. The wound care coordinator assess the patient and finds the wound is an abrasion, not a skin tear, and the location of the wound is incorrect. The wound care coordinator can redefine that wound.

  1. Sign into myAvatar.
  2. Select a client/patient.
  3. Select an episode.
  4. Select the Flowsheet console.
  5. Select the Wound Care collection.
  6. Select a wound from the wound list.
  7. Select Actions.
  8. Select Redefine Wound.
  9. Select a new Wound Type.
  10. Select Redefine.
  11. Enter a Reason For Redefining the wound.
  12. Select Save.
  13. Select Wound Type to redefine (if applicable)
  14. Select Wound Location to redefine (if applicable)
  15. Select Redefine.
    Results: The wound is listed as the redefined wound type and documentation can occur.
    • The original wound type and location is listed with the wound with Clock Icon and "Original" label.
    • The original wound type assessments can be reviewed by selecting the Up Arrow beside wound type.
    • All new assessment documentation occurs with the new defined wound type and location with Clock Icon and "Evolved" label.
