Evolve Wound

Wounds can evolve to new categories, for example a skin tear evolves to a Pressure Injury.  myAvatar Flowsheet allows for the documentation of evolved wounds and documentation progress will display.  Wounds can only be evolved by roles set with permissions in the Flowsheet Configuration.

  1. Sign into myAvatar.
  2. Select a client/patient.
  3. Select an episode.
  4. Select the Flowsheet console.
  5. Select the Wound Care collection.
  6. Select a wound from the wound list.
  7. Select Actions.
  8. Select Evolve Wound.
  9. Select a new Wound Type.
  10. Select Evolve.
    Results: The wound is listed as the evolved wound type and documentation can occur.
    • The original wound type and location is listed with the wound using the Clock Icon and "Original" label.
    • The original wound type assessments can be reviewed by selecting the Up Arrow beside wound type.
    • All new assessment documentation occurs with the new Evolve Wound Type and location with Clock Icon and "Evolved" label.
