How to enroll in Clinical Pathways
Use the Clinical Pathway Enrollment form in crisis situations to quickly alert staff of a client/individual in a pathway prior to receiving any assessments.
Note: No assessment is needed when using the Clinical Pathway Enrollment form.
| Field Name | Description |
|---|---|
| Date of Enrollment | The first identified date the client is enrolled in a pathway. |
| Pathway Name | This is a dropdown selection displaying pathways defined in Clinical Pathway Definition. |
| Primary Pathway |
This is a yes or no selection. Clients can only have one Primary Pathway defined at a time. If a patient is enrolled in more than one Clinical Pathway, only one color will show on their name in My Clients. |
