How to disenroll from Clinical Pathways
All clinical pathway disenrollment’s must be completed in the Clinical Pathway Disenrollment form. This will add an end date to the pathway and myAvatar will allow the client to be enrolled again later.
| Field Name | Description |
|---|---|
| Date of Disenrollment | The date identified to disenroll the client from the selected pathway |
| Pathway Name | Dropdown selection list of the currently enrolled pathways for the client |
| Reason for Disenrollment | Selection list for a reason why the client is getting unenrolled. This is a user defined dictionary. CWS dictionary file, 5010 dictionary data element. |
