TAR Administrative Form - Cal-PM

Record inbound Treatment Authorization Request information. Multiple TARs can be recorded under a single client episode.

  • The following message may occur for free-standing hospitals (if the value of the Location field in the Program Maintenance form for the program that the client is admitted into is set to "99 - Other Place of Service or unlisted facility") when the client is over the age of 21.
  • Ensure that the client has a Diagnosis.
  • Ensure that the client is admitted into a program that requires a CSI Admission and there is a valid value in the Legal Class field.
  • Ensure the client has Financial Eligibility identified.

  1. Go to: Avatar PM > Client Management > TAR
    Result: The Episode Information box displays episode-related data from the following forms:

  1. In the Select or Create New TAR field, select an existing TAR to edit or the Create New TAR option. Create New defaults when no TARs exist.
  2. Select Delete TAR to remove the selected TAR from the form.
    Note: Deleting the TAR Administrative file will also delete the associated information from the TAR Clinical Review, and TAR Inquiry, Resubmit or TUT forms.
  3. Select Launch TAR Report to view a report including all TARs for the client.

TAR Submission Detail

  1. The Date Received field defaults to the current date but may be changed, if needed. If this date is more than 14 days later than the Discharge Date, the TAR is considered Late and will need to be forwarded to Compliance.
  2. In the TAR Type field, select whether this is an Initial request, a 1st Level Appeal, or a 2nd Level Appeal.
  3. The Created By field defaults to the user name and ID currently logged into the application.
  4. In Type of Days field, select whether type is Acute or Admin.
    Note: If submitting a TAR Type of 1st or 2nd Level Appeal, select the file to appeal in the TAR Being Appealed field. The files in this field are filtered according to the selected type.
  5. The TAR Number field displays the complete 11 digit number based on the values entered in the TAR Prefix, TAR County Code, TAR Code and TAR Constant fields.
  6. In the TAR Prefix field, select the prefix number.
  7. In the TAR County Code field, select the code for the county.
  8. In the TAR Code field, enter the 6 digit code indicating the appropriate procedure.
  9. The TAR Constant field defaults to 5.
  10. The TAR Admit Date field defaults to the Admission Date of the episode.
  11. In the TAR Discharge Date field, enter the date of discharge.
  12. The Days Requested field displays the number of days requested.

Available Date Management

  1. The Available Dates field, populates with all of the dates that exist between the specified Admit Date and Discharge Date values. Deselect dates as necessary. If at any time the TAR Admit Date or TAR Discharge Date fields are changed, this field will update with any new dates deselected by default, and any dates that have been deleted during a TAR Clinical Review will no longer appear.
  2. Use the Date Selection field to Select or De-Select All of the available dates.
  3. The Approved and Denied fields display the count of approved and denied dates.

Hospital Information

  1. In the Default Hospital Information field, select whether the system should ​Load Default Information or Clear Default Information.
  2. In the Free-standing Hospital? field, select Yes or No.
  3. In the Hospital Name-d field, enter the name.
  4. In the NPI Number-d field, enter the NPI number.
  5. In the Address-d fields, enter the address.

TAR Compliance

  1. In the Retroactive TAR? field, select whether or not this TAR is retroactive. The Retroactive Notes field will become enabled and required.
  2. In the Retroactive Notes field, enter any comments related to the TAR.
  3. In the Were there Compliance Issues field, select Yes or No.
  4. In the Administrative Compliance Checklist Issues (Check all that apply) field, select the issues.
  5. In the Late Type field, select the type.
  6. In the Sent to Compliance Date and Compliance Cleared Date fields, enter the dates.
  7. In the Compliance Notes (Include all communications with Hospital) field, enter any compliance notes.

Ready for Review

  1. In the Date Routed to Clinician field, enter the date that the TAR was routed to the clinician for review.
  2. In the Routed By field, select the user who routed the TAR for review.
  3. In the Note to Clinician field, enter any comments to send to the clinician.

Post Review Section

  1. In the Date all TAR Paper Work obtained from Clinical Review field, enter the date that all of the paper work was received. This is required in order to finalize the review on the TAR Clinical Review form.
  2. In the Date Sent to Intermediary field, enter the date that the TAR was sent to the intermediary.
  3. In the Date Provider Notified field, enter the date that the provider was notified about the TAR.
  4. In the Annex Box No. field, enter the number.
  5. In the Annexed Date field, enter the annexed date.
  6. In the TAR Shred Date (approved TARs) field, enter the shred date.
