Practitioner Enrollment - Insurance Coverage Section - Cal-PM

Record basic malpractice practitioner insurance information as part of the Practitioner Enrollment form.

  1. Go to: Avatar PM > Practitioner > Practitioner Registration > Practitioner Enrollment
  2. From the left-hand menu, select the Insurance Coverage section.
  3. In the Malpractice Insurance Company field, enter the insurance company name.
  4. In the Malpractice Policy Number field, enter the policy number.
  5. In the Malpractice Expiration Date field, enter the date.
  6. Select Submit.
