Results Entry

This form is used to enter results header and results detail information.

  • To-Do items can be associated with results in the Notifications Setup form (RADplus Utilities).
  • Results information is populated through this form and the following:
    • An HL7 interface module
    • Netsmart CareConnect HL7 Inbound Results module
    • Results Importing form (Netsmart CareConnect)

Prerequisites:

  • This form is enabled through the Enable Results Entry registry setting.

  • If myAvatar Order Entry is not installed, the following fields will be disabled: Select Order, Deselect Order, Universal Service ID Code Lookup, Specimen Source Dictionary, Observation ID Code Lookup, and Order Result Filter.

Enter Results

  1. Go to: Avatar CWS > Results Entry > Results Entry
  2. In the Select Client screen, enter the client's name, and select.
  3. In the Add/Edit/Delete Results field:
  • Select Add.
  • Select Edit to change a result.
    Click Select Result.
    The Select Result screen displays. Choose the result, click Ok.
    If associating the result with an order, click Select Order.
    The Order Entry Client Profile Order List screen displays.
    Select the order, click Ok. The Order, and Placer Order fields display order information.
  • Select Delete to remove a result.
    Click Select Result.
    The Select Result screen displays. Choose the result, click Ok.
    Click File Header Info.
  1. To associate the result with an order, click Select Order.
    The Order Entry Client Profile Order List screen displays.
    Select the order, click Ok. The Order, and Placer Order fields display order information.
  • The Order, Placer Order # fields display the order information, when an order is selected (click Select Result).
  1. If appropriate, click Deselect Order to clear fields that have populated with the order information.
  2. In the Filler Order # field, enter the order number.
    This is the laboratory ID number for the order.
  3. In the Universal Service ID Code Lookup field, enter the code, click Process Search to select. Click Ok.
  4. In the Universal Service ID Code field, enter the test or panel for which the result is received.
  5. In the Universal Service ID Type field, enter the universal service ID code.
  6. In the Universal Service ID Value field, enter the universal service ID code description.
  7. In the Vendor Name field, enter the vendor.
  8. In the Ordering Practitioner field, enter the practitioner name, click Process Search to select. Click Ok.
    If the practitioner is not found in myAvatar , in the Ordering Practitioner field, enter the practitioner.
  9. In the Diagnostic Code field, enter the code, click Process Search to select. Click Ok.
  10. In the Relevant Clinical Information field, enter associated information.
  11. In the Reason Code field, enter the reason code, click Process Search to select. Click Ok.
  12. In the Reason Text field, enter additional reason information.
  13. In the Result Status Code field, enter the result status code.
  14. In the 'Specimen' date and time fields, enter the specimen data entry date and time information.
  15. In the Specimen Source field, enter the specimen source, and select.
  16. In the Reflex Text field, select Yes if the result is associated with a reflex test.
  17. In the Note - Client Specific field, enter any client-specific notes.
  18. In the Note - Test/Panel Specific field, enter any associated notes.
  19. In the Order Callback Phone Number field, enter the laboratory's phone number.
  20. In the 'Ordering Facility' demographic fields, enter the associated facility information.
  21. In the Results Report/Status Change Date field, enter the date the results or status was changed.
  22. In the Results Report/Status Change Time field, enter the time the results or status was changed.
  23. In the Specimen ID - Placer field enter the placer's ID.
  24. In the Specimen ID - filler field, enter the filler's ID.
  25. In the Specimen Type field, select the specimen type.
  26. Click File Header Info.
  27. Click Exit Option to close the form.
