Client-Family Educ. Assessment

Record a client and their family's interaction and how it affects their functioning.

  1. Go to Avatar CWS > Assessments > User Defined Assessments > Client-Family Educ. Assessment.
  2. In the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Click Ok.
  3. If the client has multiple episodes, select the client's episode, click Ok.
  4. In the Assessing Clinician field, enter the clinician's name and select from the list.
  5. In the Initial Assessment Date field, enter the assessment date.
  6. In the Additional Dates field, enter any additional dates that the test was administered on.
  7. Under the CLIENT'S READINESS TO LEARN column, complete the fields to define the client's Ability to Understand Verbal Instructions, Ability to Understand Written Instructions, and their Willingness to Participate in Education.
  8. Under the FAMILY READINESS TO LEARN column, complete the fields to define the family's Ability to Understand Verbal Instructions, Ability to Understand Written Instructions, and their Willingness to Participate in Education.
  9. In the Family Member(s) Involved field, enter information about the family.
  10. If applicable, enter relevant information for the Client and Family in the remaining fields.
  11. When finished, select Submit button.
