CRG Assessment

Complete a Clinically Related Group (CRG) assessment to identify an individual who is severely or persistently mentally ill or seriously emotionally disturbed.

Note: CRG assessments are completed for clients 18 years and older. For clients under 18 years old, use the TPG Assessment form.

  1. Go to: Avatar CWS > CWS Assessments > User Defined Assessments.
  2. Select the correct client and episode. 
    Note: The Client ID, Client Name, Date of Birth, Social Security #, and Principal Dx fields will auto-populate with information entered in the system previously. 
  3. In the CRG Date field, enter the assessment date. 
  4. In the Default Diagnosis field, select the client's diagnosis.
    Note: Diagnosis information is entered in the Diagnosis form (myAvatar PM).
  5. In the Dual Principal field, search for and select the dual principal. 
  6. For the Current GAF, rate how serious the patient's mental illness is currently. 
  7. For the Highest GAF, rate how serious the patient's mental illness is/was at its highest point. 
  8. For the Lowest GAF, rate how serious the patient's mental illness is/was at its lowest point. 
  9. In the Assessment Type field, select the type of assessment.
  10. In the BHO ID Number field, enter the BHO ID number. 
  11.  In the Activities of Daily Living field, choose a ranking value. 
  12. In the Comments field, enter comments about the client's activities of daily living. 
  13. In the Interpersonal Functioning field, choose a ranking value. 
  14. In the Comments field, enter comments about the client's interpersonal functioning. 
  15. In the Concentration, Task Performance, and Place field, choose a ranking value. 
  16. In the Comments field, enter comments about the client's concentration, task performance, and place. 
  17. In the Adaptation to Change field, choose a ranking value. 
  18. In the Comment field, enter comments about the client's adaptation to change. 
  19. For the Severity of Impairment, select Yes or No
  20. For the Duration, select Yes or No
  21. In the Formerly Severe field, select Yes or No
  22. In the Needs Services to Prevent Relapse field, select Yes or No
  23. In the Determination of Consumer Group field, select a determination. 
  24. In the Reason for Assessment field, select the reason. 
  25. In the Date of Request/Circumstance field, enter the date. 
  26. In the Date of Initial CRG Assessment field, enter the date. 
  27. In the Consumer Information Indicator field, choose an indicator. 
  28. In the Notes field, enter corresponding notes. 
  29. In the Program Code field, select a program code. 
  30. In the Assessing Clinician field, search for and select the clinician. 
  31. Select Submit.
