TPG Assessment

Target population group (TPG) assessments identify individuals who are severely or persistently mentally ill or seriously emotionally disturbed. CRG assessments are completed for clients under age 18. For clients over 18 years old, use the CRG Assessment form.

  1. Go to: Avatar CWS > Assessments > User Defined Assessments
  2. In the Select Client screen, enter the client ID in the Client Name/ID field. Click Select Client Name/ID Search. select OK.
  3. If the client has multiple episodes, select the client's episode, select Ok. The TPG Assessment opens. 
  4. The Client ID #, Client Name, Date of Birth, and Social Security will automatically populate with client data. 
  5. In the TPG Date field, enter the assessment date.
  6. If appropriate, select the default diagnosis in the Default Diagnosis form.
    Result: Diagnosis information is entered in the Diagnosis form (Avatar PM).
  7. The Principal Dx field will dynamically display. 
  8. In the Current GAF field, enter a value. 
  9. In the Highest GAF field, enter a value. 
  10. In the Lowest GAF field, enter a value. 
  11. In the Assessment Type field select Initial or Follow-Up. 
  12. Enter a number in the BHO ID Number field. 
  13. In the Is Current GAF Score or Lowest GAF Score Below 51 field, select Yes or No. 
  14. Select whether the client meets SED criteria in the Meets SED Criteria field. 
  15. Make the appropriate choices (Yes or No) in the Environment Issues, Trauma Issues, Social Skills Issues, Abuse/Neglect Issues and the Children at Risk of SED fields. 
  16. In the Determination of Consumer Group field, select a value. 
  17. In the Reason for Assessment field, select a reason. 
  18. Enter a date in the Date of Request/Circumstance field in the MM/DD/YYYY format, using the calendar icon, or by using the T or Y buttons for today or yesterday. 
  19. Enter a date in the Date of Initial TPG Assessment field in the MM/DD/YYYY format, using the calendar icon, or by using the T or Y buttons for today or yesterday. 
  20. Select a code in the Program Code field.
  21. In the TennCare Provider Clinician ID field, search for and select the provider. 
  22. Once finished, select Submit.
