Spill/RTI/Exception Doses

Spill / RTI - the form allows the user to track if medication was spilled and therefore needs to be re-dispensed, or medication needs to be returned to inventory. If a dosage is logged as spilled, the dispense record is backed out and the client can be re-dispensed. If a dosage needs to be returned to inventory, a selection is made to return it to pre-pour (if the Pre-pour option was used to create the dose) or to return it to inventory. The inventory is then adjusted. 


Exception doses are also able to be created within this option based on adjustments to the doctors order. 

To Return a Dose to Inventory

From the Spill/RTI/Exception Dose form: 

  1. Choose the Spill/RTI option from the left-hand menu.
  2. Select the episode for the dosage spilled and enter your password.
  3. Choose the date that the dose was spilled or returned to inventory and the date that the dose was dispensed.
  4. The Dose record will populate with doses dispensed on the specified date.
  5. Choose the appropriate dose, input a reason, and select RTI or Spill. If you are performing an RTI, (The Inventory Number To Return To will become mandatory). 
  6. Use the drop down menu to specify which bottle the inventory is being returned.
  7. Click to ‘File’ save the change. 

About Exception Dosage

Exceptions, or added dosage, can be administered to a client with orders by a physician. If an exception dose is to be administered, the care giver can do that via the tab in Fast Dose Dispensing or Spill/RTI/Exception form. Exceptions are ordered in addition to a daily dose, not instead of. For example, if a client comes in and the physician see them and determines that the client will receive an extra 10mgs of methadone today based on his or her condition, the care giver will administer the original dose and then an exception dose of 10 mgs. 

To Enter an Exception Dosage

From the Spill/RTI/Exception Dose form: 

  1. Choose the Exception Dosage option from the left-hand menu.
  2. Enter your password.
  3. Choose the medication and dispense, start, and end dates. These should typically all be the date of dispensing (T for today). ‘No’ is the default response for Dispense First Dose As Take Out. Last Dispensed Amount will automatically pre-fill. Input the ordered exception dose (in mgs) into the Amount To Dispense field, choose the ordering practitioner, and appropriate notes for this order.
  4. When all fields are complete and there is a container to catch the medication under the pump, click ‘Dispense’. This will pull up the dispensing screen and the exception dose will be administered like a daily dose. 

After you select the dispense button you will be able to see that only 20mgs is beings dispensed for a single dose. Below in the red boxes highlight these items.
