Phase Input
Phase Input can be used to control the phase or level of care that the client is associated with. Example of phases may be Detox, Rehab, Maintenance, etc. The form allows for linking the phases to a date range so that the phase history can be viewed. The phase input can be used in the Care Plan Activity Maintenance / Add Auto Scheduling Parameters. For example a client in a Detox phase may be required to provide a weekly urinalysis and once they move through to Maintenace phase they may be dropped down to a monthly urinalysis requirement.
Add a Client to a Phase
“Begin Date”, “Phase” and “Reason” fields are all required fields. The “End Date” field is not required but data will need to be entered in if a client is moving to another phase. No client can be entered in two phases at once. The “Phase” field is a drop-down dictionary. The Phase dictionary is defined by your organization during implementation. After an organization's go-live, the system administrator is responsible for all updates.
