Cal-OMS Youth/Detox Discharge - Cal-PM
Discharge a youth (17 years of age or younger) and/or client enrolled in a California Outcomes Measurement System (Cal-OMS) Detox program. A program is determined to be a Detox Program when:
- Substance Abuse Program (Cal-OMS) = Yes
- County ID (CalOMS) is present
- Provider ID (CalOMS) is present
- Licensed For Methadone and/or LAAM = Yes
- Cal-OMS Type of Service is one of the following:
- Nonresidential/Outpatient Detoxification
- Residential Detoxification (hospital)
- Residential Detoxification (non-hospital)
Prerequisite:
- The client must have been assigned to a Cal-OMS treatment program through the Cal-OMS Admission form.
- A client must be discharged from a treatment program in either the Discharge, or Discharge (Outpatient) form.
- Either the Enable LA County Reporting Requirements, or the Enable Non LA County Requirements registry setting must be enabled to process Cal-OMS data collection and submission.
- Go to: Avatar PM > Client Management > Client Information > Cal-OMS Youth/Detox Discharge
- In the Select Client screen, search for and select the client by name or ID.
- The Episode Selection pre-display shows if the client has multiple episodes.
- Select an episode, click OK.
- In the Discharge Status field, select the client's discharge status.
- In the Consent field, select Yes if the client has given consent to be contacted in the future.
- In the Disability field, select the client disability.
- In the Current First Name field, enter the client’s first name if different from the birth name. Enter "99904" if the client is unable to provide an answer.
- In the Current Last Name field, enter the client’s last name if different from the birth name. Enter "99904" if the client is unable to provide an answer.
- In the Primary Drug (Code) field, select the client’s primary drug of use. If Other (Specify) is selected, enter the name of the client’s primary drug in the Primary Drug Name field.
- In the Primary Drug Frequency field, enter the drug use frequency.
- In the Primary Drug Route of Administration field, select the client’s primary drug route.
- In the Primary Drug Age of First Use field, enter the client’s age at the time of first drug use.
- In the Secondary Drug (Code) field, select the client’s secondary drug of use.
- Selecting None enters "99902" in the Secondary Drug Frequency, Secondary Drug Age Of First Use, and Alcohol Frequency fields.
- If Other (Specify) is selected, enter the name of the client’s secondary drug in the Secondary Drug Name field.
- In the Secondary Drug Frequency field, enter the drug use frequency.
- In the Secondary Drug Route of Administration field, select the client’s secondary drug route.
- In the Alcohol Frequency field, enter the frequency of alcohol use, in the last 30 days. This field is used when the primary and secondary drugs are not alcohol.
- In the Pregnant At Admission field, select Yes if the client was pregnant at the time of admission.
- In the Employment Status field, select the client’s employment status.
- In the Enrolled in School field, select Yes if the client is enrolled in school.
- In the Number of Arrests Last 30 Days field, enter the number.
- In the Mental Illness field, select Yes if the client has mental illness.
- In the Social Support field, enter the number of days in the last 30 days the client has participated in social support recovery activities.
- In the Current Living Arrangements field, select the client’s current living arrangement.
- In the ZIP Code at Current Residence field, enter the ZIP code for the client's current residence.
- Click Submit.
