Site Inquiry

View or print a report containing all site information entered in the Site Registration and Site Termination forms.


  1. Go to: Avatar PM > Appointment Scheduling > Site Management
  2. In the Site Option drop down menu, select the type of inquiry being made.
  3. In the Individual Or All Sites field, select if the inquiry is for an individual site or for all sites.
    • If Individual is selected, enter the Site Name or ID# and select the appropriate site from the list.
  4. Enter the Inquiry Start Date and Inquiry End Date.
  5. Select Process.
