Clinical Section
Use this section to enter medical diagnosis information and specify if the appointment requires follow-up.
- All follow-up appointment field displays are controlled through the Enable Follow-Up Appointment Functionality registry setting.
- Also, the Referring Provider, Referring Practitioner, and Diagnosis fields are controlled through the Scheduling Forms - Fields to Include registry setting.
- If applicable, enter the Referring Provider Name or ID#, and select the appropriate staff member from the list.
- If applicable, enter the Referring Practitioner Name or ID#, and select the appropriate staff member from the list.
- Select the Active Diagnosis from the drop-down list.
- In the Medical Diagnosis 1 field, enter the ICD-9 description or code associated with the appointment.
- If applicable, enter another Active Diagnosis and Medical Diagnosis 2.
- In the Appointment Requires Follow-Up field, select Yes if a follow-up appointment is recommended. If Yes, enter the Follow-Up Appointment Due Date.
- In the Follow-Up Appointment For drop-down list, select the appointment for which the follow-up visit is being made. The drop-down list will contain all scheduled appointments for the client.
- In the Reason For Follow-Up field, enter any notes regarding the purpose of the follow-up visit.
- In the Follow-Up Status field, select the current status of the follow-up appointment.
- Click Submit.
