Client Charge Input With Medical Diagnosis Section - Cal-PM

Enter the client charge and medical diagnosis information.


  1. Go to: Avatar PM > Services > Ancillary/Ambulatory Services > Client Charge Input With Medical Diagnosis Section 
  2. In the Date Of Service field, enter the service date.
  3. In the Client ID field, enter the client name, click the Search button, and select the corresponding entry.
  4. In the Episode Number field, select the client episode.
  5. In the Program field, select the client program.
  6. In the Service Code field, enter the service code, click the Search button, and select the corresponding entry.
    Note: The service must be required by a client, or a client and practitioner (Service Required By field, Service Codes form).
  7. In the Practitioner field, enter the practitioner name, click the Search button, and select the corresponding entry.
  8. Select Display Managed Care Authorization Data.
    Result: The Managed Care Authorization Data screen displays managed care authorizations for the client, service code, and date of service.
  9. In the Duration (Minutes) field, enter the service duration.
  10. In the Location field, select the service location.
  11. In the Co-Practitioner field, enter the practitioner name, click the Search button, and select the corresponding entry.
  12. In the Co-Practitioner Duration (Minutes) field, enter the duration.
  13. The Cost of Service field displays the service fee (set up in the Service Codes and Service Fee Maintenance forms).
  14. In the Active Medical Diagnosis field, select the medical diagnosis.
    Note: This list contains medical diagnoses associated with the client (Medical Diagnosis form).
  15. In the Diagnosis fields, enter the ICD-10 code, select the Search button, and then choose the corresponding entry.
  16. In the Evidence-Based Practices / Service Strategies (CSI) field, select the service strategies.
    Note: This field provides additional service information.
  17. In the Additional Service Information field, select the applicable codes.
  18. In the Psychotherapy Add-On Duration field, enter the add-on duration in minutes.
  19. In the Add-On Notes field, enter any additional information related to the add-on
  20. Select Submit.
