Practitioner Termination - PM
If a practitioner leaves your organization and you need to remove their access to myAvatar you can terminate that user's role as an active practitioner using the Practitioner Termination form. There are a few things to be aware of in this process:
- A practitioner can be registered again in the Practitioner Enrollment form.
- Terminating a practitioner is not the same as deactivating the myAvatar user. The user will continue to be able to access the system until their username has been deactivated in the User Definition form.
- You are able to terminate practitioners who have existing open appointments and choose to place those appointments on hold. Once placed on hold, the appointments cannot be verified or posted until the appointment is updated.
This functionality requires myAvatar PM 2017 Update 60.
Prerequisite: A practitioner must be enrolled in myAvatar through the Practitioner Enrollment form.

Go to: myAvatar PM > Practitioner > Practitioner Registration > Practitioner Termination - In the Select Staff screen, enter the staff name or ID, and select. Click Select.
- In the Termination Date field, enter the practitioner termination date. The termination date cannot be changed once this form is submitted.
- In the Reason For Termination field, select the termination reason.
- Click the Check here if you would like to hide this staff on the "Scheduling Calendar" box to prevent this practitioner from appearing on the Scheduling Calendar. This field is only available if myAvatar Appointment Scheduling is installed.
- Click Submit.
