Follow Up Entry (Follow Up Entry Section) - PM

Edit the follow-up note that was selected in the previous section of this form.

  1. In the Contact Date and Contact Time fields, enter the contact date and time.
  2. In the Episode Number field, select the client episode.
  3. In the Guarantor field, select the guarantor.
    Note: Guarantors are setup in the Financial Eligibility or Fast Financial Eligibility form.
  4. In the Guarantor Representative field, enter the guarantor representative name or ID, and select.
    Note: This is the myAvatar User who is performing the data entry. Only active myAvatar users (defined in User Definition) will be available for selection in this field.
  5. In the Collection Note Type field, select the collection note type.
  6. In the Contact Name field, enter the name in the format LAST, FIRST.
  7. In the Contact Phone field, enter the phone number.
  8. In the Next Contact Date and Next Contact Time fields, enter the next contact date and time.
  9. In the Follow Up Note field, enter follow up note information as appropriate.
  10. Select File Follow Up Entry.
