First and last name of the patient's primary physician.

Master ID

Patient's master ID specified in Patient > General > Basic.

Start of Care Date

The date when the patient started receiving care.

Diagnoses

Diagnoses, diagnoses codes, and effective dates, and the primary diagnosis of the patient as of the current date specified in Patient > General > Diagnoses.

Medications

Medications, dose, frequency, route of intake prescribed for the patient, effective dates, and ordering physician specified in Patient > Clinical > Medications.

Visit Frequency

Information on the frequency of the planned visits, effective dates, and ordering physician specified in Patient > Clinical> Visit Frequency.

Interventions

Intervention descriptions, performing discipline, effective dates, and ordering physician specified in Patient > Clinical > Care Plans.

Clinical Notes

Clinical notes entered for the patient, discipline, effective dates, and ordering physician specified in Patient > Clinical > Clinical Notes.

General Clinical

Patient's information on diet, allergies, DME, safety, functional limits, activities permitted, mental status, advance directives, and prognosis specified in Patient > Clinical > General Clinical.
