Certification of Terminal Illness window (Orders)

Path: Orders>Demand Documents>Certification of Terminal Illness

Prerequisite: You must have appropriate privileges from the Netsmart Homecare administrator.

With the Certification of Terminal Illness window, you can preview and print generated CTI documents for selected patients. For the forms with New status, if you have corresponding privileges, you can edit benefit periods, specify and edit MD CTI. Specifying an attending MD is required for CTI form A and optional for CTI form B.

By printing a CTI, you make it available for further processing.

You can reprint (see Reprint CTIs (Orders or Patient)) or inactivate (see IReprint CTIs (Orders or Patient)) CTIs if necessary.

For each selected patient, you can see the status of all CTI forms, CTI physician, CTI dates, date of printing, and the date of approval or rejection if the form was processed in Transactions>General>Process CTIs.

If any changes were made to the printed CTI, it should be reprinted manually (see Print Certification of Terminal Illness (CTI) (Orders).

Note: If the CTI chain is changed, forms with the Pending status are inactivated automatically and new ones are generated.

MD CTI ID

Prerequisite: To enter this physician, you must be granted the appropriate security privilege.

Physician certifying terminal illness. MD CTI may be prefilled from the MD CTI field (Patient>General>Admissions & Status) if the physician entered there is not NP. If needed, you can change the prefilled MD CTI.

Entering MD CTI is required to preview the CTI form with New status. This physician is reported on NOEs and claims in the following places:

  • FL 78 and/or FL 79 in the UB-04 form only if the Print CTI physician's information with NPI print variation is selected in Administration>Financial>Insurance Codes for these locators.

    MD CTI from the most recent active CTI document that covers the claim from date is reported together with the individual NPI. If the MD CTI ID field is empty or the same CTI MD is entered in the Admissions & Status and Certification of Terminal Illness windows, physician from the MD CTI field and the value from the NPI CTI field (Patient>General>Admissions & Status) are reported.
  • Loop 2310F for Medicare hospice electronic claims.
