Print Certification of Terminal Illness (CTI) (Orders)

Path: Orders>Demand Documents>Certification of Terminal Illness

Prerequisite: To print or reprint CTIs, you need to have the appropriate privilege granted by the Netsmart Homecare administrator.

By printing a CTI, you change its status to Pending and make it available for signature and processing.

  1. Go to Orders>Demand Documents>Certification of Terminal Illness.
  2. On the Define tab, click Select Patients, and add the patients for whom you need to print CTIs. 
    All the generated CTI forms for the selected patients are listed. For the CTI physicians using Physician Portal, the documents are displayed in the Electronic Signature section.
  3. If needed, view the CTIs from inactive admissions by selecting the Show Prior Admissions check box, and the inactivated and rejected CTIs by selecting the Show Inactive/Rejected CTIs check box.
  4. Select the CTIs that you want to print for further signature (applies to the New status only).
  5. For the forms with New status, enter MD CTI. 
    Note: If MD CTI that is not a nurse practitioner is specified in Patient>General>Admissions & Status, this MD CTI is automatically entered in the Certification of Terminal Illness window.
  6. For CTI form A and optionally for CTI form B, specify the attending MD, and then specify who signs the form first in the First send to field.
  7. If needed, edit the benefit period.
  8. To preview the CTIs, click the Preview tab. 
    If you change the criteria on the Define tab after preview, you must close and then open the Preview tab again to reflect your changes.
  9. If the CTIs are ready for signature, change their status to Pending:
    1. For paper signature, print the CTIs using the Preview toolbar.
    2. For electronic signature, click Set Pending, and then click Save.
  10. CTIs appear on Physician Portal for review and signature.
