Use the HIV History form in myEvolv to document a client's HIV status history.

Menu paths
  • Client > Client Information > Health Information > HIV History
  • Referral > Agency Referral > Health Information > HIV History
  • Program Referrals > Program Referral > Health Information > HIV History
  • Referral Application > Application Data for Providers > Health Information > HIV History
  1. Select from the modules above and navigate to the appropriate path.
  2. Search for and select a client.
  3. In the toolbar, click New Event > HIV Status.
  4. In the HIV Status History dialog, enter any of the relevant information. The following information is required:
    • Status Date - Click the calendar to enter a date.
    • Status - Click the magnifying glass to select an item from the HIV Status Values dialog. Examples of possible selections are HIV Affected, HIV Infected Asymptomatic, or Unknown.
    • Symptom - Click the magnifying glass to select an item from the HIV Symptoms dialog. 
  5. Click Save.
