Create an incident
When you create an incident in myEvolv you identify the type of incident, describe what occurred during the incident, and identify the participants.
After you save an incident, you can access the incident even if you do not have access to all incidents.
If your navigation access allows access to the New button, click New to see a list of all active incident types in your system. Each incident type can have its own form.
- Go to: Incidents > Incidents > Incidents > Incident.
- In the top right corner, click Create Incident and select the type of incident from the menu.
- Complete the appropriate fields. The following fields are generally required.
- Type - The type of incident.
- Location - The place where the incident occurred.
- Person filling out form - The name of the person logged in.
- Actual Date - The date the incident occurred.
- Participants - The people involved in the incident. There must be at least one participant indicated as a report holder.
- Description - Details of what occurred during the incident.
- In the left pane, select the appropriate areas and enter additional information, depending on the type of incident.
- Select Save.
