Create a Permanency Plan Goal

Typically, lines of service that are for children in out-of-home services (such as foster care, residential, independent living, and adoption) require that the client have a Permanency Plan Goal (PPG). This serves as the overall goal of the placement with a target date. This goal can be changed over time. In myEvolv most workflow setups and event scheduling for this line of service use the PPG as a qualifier for scheduling events. The effective dates of PPGs are often used as the trigger for scheduling, especially for adoption events.

There are two types of Permanency Plan Goals (PPGs):

  • Primary – Only one primary plan can be active at a time.
  • Concurrent – Used when agencies want to have an alternative plan in case the primary plan fails or is changed in the very near future. This can be used to aid in ASFA regulations when a worker knows that a foster client is going to be changed to adoption in the very near future. The worker can make a concurrent plan of adoption to get a jump-start on those events.

Note: Only one Concurrent plan can be active at a time.

Add a primary permanency plan goal 

Only one primary permanency plan goal can be active at a time.

  1. Go to: Client Client Information Critical Information > Permanency Plan Goals.
  2. Search for and select a client.
  3. In the toolbar, click New Event Primary Permanency Plan.
  4. In the Primary Permanency Plan Goal dialog, complete the relevant fields. The following fields are required:
  • PPG - Search for and select a permanency plan goal from the PPG Definitions Table.
  • Date Approved - Click the calendar to select a date.
  1. Click Save.

Add a concurrent permanency plan goal 

Only one concurrent permanency plan goal can be active at a time.

  1. Go to: Client Client Information Critical Information > Permanency Plan Goals.
  2. Search for and select a client.
  3. In the toolbar, click New Event Concurrent Permanency Plan.
  4. In the Concurrent Permanency Plan Goal dialog, complete the relevant fields. The following fields are required:
  • PPG - Search for and select a permanency plan goal from the PPG Definitions Table.
  • Date Approved - Click the calendar to select a date.
  1. Click Save.
