Agency referrals report
In myEvolv go to: Reports > Referrals > Agency Referrals to run the Agency Referrals report that shows the type of referral, clients referred to your agency, the reason for the referral, and related information.
System administrator: For clinical and supervisory reports, there is a setting to mask the entire Social Security number or show only the last four digits of the Social Security number (Setup > System > EvolvCS System Setup > System Settings/Preferences and SSN Preferences).

Referrals to the Agency |
This report has optional From and Through dates, and Report Selections to present the information in aggregate form, by referral source, or by referral type. Dependent upon Report Selection, the report will present: the referral source, Source type, client name, referral date, staff who processed, where processed, referral reason, priority, and outcome. Even the non-aggregate reports will show totals accepted, pending, denied/rejected, withdrawn, and resulted in enrollment. |
Program Oversight |
This report can be useful for program planning (for example, if multiple referrals are rejected because the agency cannot meet the need) as well as monitoring waiting lists and reviewing referrals. |
