Add Physical Characteristics and Vitals

Track the history of a client's physical appearance. For example, children in foster care mature and your agency may opt to add a new record, which can include a photo, each year. This form contains vital information, but also includes additional physical information. You can access this information in multiple places in myEvolv.

Menu paths

  • Client > Client Information > Personal Information > Physical Characteristics
  • Client > Client Information > Health Information > Vitals
  • Referral > Agency Referral > Referral Information > Physical Characteristics
  • Program Referrals > Program Referrals > Referral Information > Physical Characteristics
  • Referral Application > Application Data for Providers > Application Information > Physical Characteristics
  1. Go to one of the above modules, then search for and select a client or a referral.
  2. In the toolbar, select Add New, then Physical Characteristics.
    Result: The Physical Characteristics dialog opens.
  3. Enter information in the appropriate fields, including a client picture.
  4. In the Physical Characteristics dialog, select Save.
