Upload a client picture to physical characteristics
Add a client picture as part of their physical characteristics information on file in myEvolv. Unlike the photo in the Appearance section of the Demographics form set used for clerical and identification purposes, the picture saved here is for clinical purposes.
Menu paths
- Client > Client Information > Personal Information > Physical Characteristics
- Client > Client Information > Health Information > Vitals
- Referral > Agency Referral > Referral Information > Physical Characteristics
- Program Referrals > Program Referrals > Referral Information > Physical Characteristics
- Referral Application > Application Data for Providers > Application Information > Physical Characteristics
- Go to one of the above modules, then search for and select a client or a referral.
- In the toolbar, select Add New then Physical Characteristics.
Result: The Physical Characteristics form opens. - Under Picture select Click to Upload Image and choose an option:
- Select Upload File then Choose File. Select the file then choose Upload File.
OR - Select Capture from Camera. The Capture Image dialog displays the client's image. Select Capture Image then Upload File.
- Select Upload File then Choose File. Select the file then choose Upload File.
- Select Save.
