View and Modify Program Referral Information
After completing a New referral, you can update and change a person's referral information in this area.

Details
You can make modifications for the following form set members:
- Referral Information - Provides basic referral information such as where the referral came from, date referral came in, worker responsible, and so on.
- Demographics - Provides personal information, such as DOB, Address, SSN, and so forth.
- Physical Characteristics - Allows you to track changes in the person's physical appearance over time.
- Benefit Assignments - Tracks any benefits the person may receive. These can include private insurance, Medicaid, Medicare, County Sponsorship, Self Payers, and so on.
- Employment History - Lists employment history of the referral, if any.
- Income Information - Provides income information of the referral, if available.
- Schools Attended - Lists non-agency affiliated schools.
- Degrees - Degrees obtained by the referral, if any.
- Legal History - Legal history, such as detentions, incarcerations, and so forth.
- Eligibilities / Classifications - Lists any potential barriers to treatment.
- Complete Info - Provides additional information about the referral to help complete the process.
- Referral Forms - View various referral forms that cover duration, date, location, title, and other criteria for each event.
The Referral Face Sheet shows a snapshot of all referral information for the selected entity in the module. This information is read-only and can be printed by staff who has access to it.
The Data Collection form is a collection of Sub Forms that allow a user to enter Referral data in a single form. This can be used as an alternative to navigating the above formset members and entering information one area at a time.
Modify Referral Information
After a referral is entered, to update the basic referral information, complete the following steps.
- In the myEvolv toolbar, click the Program Referrals button.
- In the Navigation pane, expand Referral Information > Referral Information.
- In the toolbar, click the Select Referral button.
- In the Referrals to Program dialog, search for and select a referral.
- In the Programs to Referrals Referral Information screen, in the Referral Information tab, click the Edit Form in New Window button.
- In the Referral to Program from Outside Source dialog, make any applicable changes.
- Click Save.
