Note: This companion guide is a crosswalk of how myEvolv maps data to certain boxes in the UB04 output. The output was designed for specific client needs and is not 100% in sync with 837i outputs. If a non Medicaid (not managed care) funder requires either a non-populated box or something different in a populated box, please submit a support ticket in inquire about an LOE for custom work.
