Managing Medications without OrderConnect

Learn how to manage and modify medications in myEvolv. This is helpful in situations where:

  • Your Agency is not using OrderConnect to manage medications.
  • Your Agency has a need to identify medications prescribed by providers other than those within the Agency.
  • Your Agency has a need to identify historical medications the client had been previously prescribed.
Category

  • This is the overarching category for the medications; the types of medications.
Controlled Substance Indicators

  • This allows the Agency to define the level of potential for abuse, and identify which of those levels indicate a controlled substance.
Drug Form / Dose Form

  • This indicates the way the medication is presented (cream vs pill, for example).
Drug Dose Amounts

  • This indicates how the medication should be administered; orally vs applied to the skin, as an example.
Medication Frequency

  • This will indicate how often the medication should be administered or taken by the individual.
How these items combine to dictate the medication administration

The combination of the Dose Form, Drug Dose amounts, Product Strength, How Administered, and Medication Frequency will indicate to the user exactly how much of the medication the individual should receive. For example: the person is prescribed 4mg pills, and they should take 2 of them orally three times per day. The ‘4mg’ will come from the Product Strength, the ‘pills’ will come from Dose Form, the ‘2’ will come from the Drug Dose Amounts, the ‘orally’ will come from How Administered, and the ‘three times per day’ will come from Medication Frequency.

Substance Frequency

  • This is different from the Medication Frequency. This area relates specifically to Substance Use. This table appears as the options for users when they are identifying substances the individual client uses or has used. This allows differentiation, and prevents options such as ‘no use during the month before’ from showing up in the prescribed medication area.
Substance Status Codes

Similar to the ‘Substance Frequency’ area, this is not specifically related to medications, however is included in this guide in order to cover the entirety of this Formset.

  • This area allows a user to identify the client’s current relationship to their substances of choice. You can identify whether use has increased, decreased, remained the same, discontinued, etc. These options only appear in the substance use section of the chart, as they are not relevant to prescribed medications.
eMAR Outcome Reasons

  • This area dictates the options available for users when they are completing the eMAR (electronic Medication Administration Record). If a medication is not administered, for example, the user is prompted to choose a reason why it was not administered. This area allows the Agency to dictate the options available to the user when that is chosen, and to differentiate the options available when ‘dose changed’ is identified, vs when ‘Not Administered’ is identified.

Identifying Medications

The first step in identifying a medication is to identify the category. If the available options are not sufficient for the Agency’s needs, then a new category can be identified.

Choose Select a Category to search for and select from the existing drug category list. Select New to create a new drug category.

Medication Listing

The medications are filed into Categories. Once a Category is chosen, the medications will be listed. New Medications can be added here, as well.

The tables discussed in the previous sections will be relevant here. It is required to identify the Name, Code, Category, and Controlled Substance indicator.

It is possible, but not required, to also identify the Route, Dose Form, Product Strength, and other codes and product identification.

Note that this will be the route, form, and strength each time this medication is chosen, and cannot be modified. The use of these fields should be carefully considered. It is most useful when these options never change, and each person who is prescribed this medication will take it in the same exact way.

In addition, note that if the Agency will be using the eMAR, then the user will still be required to identify the delivery, dose, form and frequency in the separate fields in the actual client chart.

End User Experience of Medications

  • This is where the user will identify the medications that are prescribed to the client.
  • Under the Medication Details, select the lowercase i next to the drug name to be redirected to MedlinePlus. Additional information about the medication is listed on this website which is managed by the US National Library of Medicine.
  • Also, note that there are separate fields for End Date, and Date Discontinued. This will identify the difference between the last date the person ingested the medication vs the date the doctor officially discontinued the medication, if they are different.
  • There are also different fields to identify the Start Date vs the First Administration Date. For example, if the doctor prescribes the medication on one date, but states that the person should not start ingesting the medication until the following Monday, these dates can be differentiated.
