Medication Template Setup for System Administrators

Medication Templates allow for the identification of medications which are available to the majority of Individuals receiving services.

Note: This is different from actually prescribing the medications to individuals. Templates are used primarily in residential settings (although they can be used elsewhere) to identify medications such as acetaminophen, antacids, multivitamins, and so on which tend to pertain to all Individuals receiving services.

This article covers how the System Administrator defines Medication Templates within the setup area of myEvolv. To obtain information on how End Users will use these Medication Templates to add medications to the chart of an individual receiving services, reference the article ‘Adding a Medication Template to the Chart of an Individual Receiving Services'.

Set Up the Template

  1. Navigate to: Agency Setup > Other Setups > Medication Template > Details.
  2. In the toolbar, click Select Template.

System Result: If this is the first template being identified, the list will be empty.

  1. In order to identify a new Template, you must perform a search  to activate the New button.
  2. Name the Template.

Note: Use a name which will make intuitive sense to the end user (the Nurse or Front Line Staff Member who will actually add this Template to the record of the Individual receiving services)

  1. Save.
  2. To add the medications to the template, click New.
  3. Enter the information for Drug, Delivery, Dose, Form, Frequency, Times of Day for Dispension, and Quantity. Each of these fields are required.

Notes

  • There is no field for strength. It is assumed that the strength is the same amongst all persons, because the medication will be coming from a Program Wide container (such as an over the counter bottle of Acetaminophen that is distributed to individuals as necessary).
  • Quantity refers to the number of doses present in the bottle upon purchase.
  1. Select + Add

Note: In order to add the medication, the information you enter for Frequency and Times of the Day for Dispension must be compatible. For example, if the Frequency entered is every 12 hours, the system requires 2 Times of the Day for Dispension. Selecting As Needed removes that requirement.

  1. Repeat Steps 6 – 8 for each medication desired, until a full list is identified.
  2. If a change is necessary after the medication has been added to the template, click on the row which contains the medication, and that row will become available allowing changes to be made. Ensure you click Update after making the necessary changes.
  3. Click Save.

Note: If you click Cancel prior to Save, all medications entered since the last save will be removed from the template.
