User DSM Table Import File Format

Location (Column)	Description	Comments
1	Diagnosis Code	Required 10 characters maximum. Use only alphanumeric characters, or a period.
2	Diagnosis Description	Required. Cannot contain '^', '&', '~', '\|', ':', '=" Descriptions greater than 80 characters are shortened to 80 characters. Description is converted to uppercase.
3	Axis 1	'Y' diagnosis is valid for Axis 1. ‘N’ diagnosis is not valid for Axis 1. Blank diagnosis is not valid for Axis 1. If Axis 2 is 'N' or blank, this column must be 'Y'.
4	Axis 2	'Y' diagnosis is valid for Axis 2. ‘N’ diagnosis is not valid for Axis 2. Blank diagnosis is not valid for Axis 2. If Axis 1 is 'N' or blank, this column must be 'Y'.
5	Associated DSM Code	Required. Must be valid DSM code.
6	Associated ICD9 Code	Required. Must be valid ICD9 code.
7	Start Date	Use the format MM/DD/YYYY Can be blank. Invalid if after end date.
8	End date	Use the format MM/DD/YYYY Can be blank. Invalid if before start date.
9	Active	Column can be blank. ‘Y’ active. 'N' not active.

6	DRG Code	Use existing DRG code.
7	Other DRG Code(s)	'&' delimited list of existing DRG codes. For example, Code1&Code2&Code3
8	Requires E-Code For Billing	'Y' diagnosis should be billed with an E-code. 'N' diagnosis should not be billed with an E-code. If blank diagnosis should not be billed with an E-code.
