Modify the layout of a myAvatar form with Form Designer.

This form provides you with the ability to:

  • Move text and data elements.
  • Hide non-required data elements.
  • Change data elements labels.
  • Resize label text, and data entry fields.
  • Use HTML to modify label display.

Note: Changes made in this form affect the system code the user is logged into. Making changes to an myAvatar form may affect the display of future enhancements Netsmart develops for that form.

  • Adding a field in Form Definition adds the field to a new subsection in the Form Designer edits for that section.
  • Removing a field in Form Definition removes the field in the Form Designer edits for that section.
  • When a registry setting that controls the display of a field is set to remove the field, the field is also removed in the Form Designer edits for that section.
  • Required objects cannot be hidden.

  1. Go to: RADplus Utilities > Modeling > Form Designer

  2. If appropriate, click Import Form Designer Copy. The Form Designer Import dialog displays. Navigate to the XML file, click Open.

  3. In the Forms field, select the form to edit.

  4. In the Tabs field, select the section of the form to edit.
    The Tab Name displays the title of the section. Edit the name if applicable.

  5. In the Other System Codes to File Form Designer Changes to field, select the additional system codes to include in this change.
    Note: This makes the changes made to the form available in those system codes. Changes are not made to forms not located in the selected system code, or modeled forms that have not been copied to the selected system code.

Modify a Form
Revert to a Previous Version

Revert To Other Form Designer Copy rules.

  1. In the Revert To Other Form Designer Copy field, select Yes to change the form back to an earlier version. 
    Reverting removes edits made since the earlier version was filed.

  2. In the Select Copy to Revert to field, select the form version. This field displays the available versions of this form along with which user or event made the changes to the form's layout.

Export Form

  1. In the Export All Tabs or Selected Tab field, choose the form information to export to an XML file.

  • Select All Sections to export information from all form sections.

  • Choose Selected Section to export the information from only the section selected in the Tabs field.
  1. Click Export Form Designer Copy. The Form Designer Export dialog displays. Navigate to the directory, click Save.
    Any myAvatar form can be exported, including product forms, and RADplus Utilities modeled forms.
Documentation

  • In the Online Documentation URL field, enter the URL to override the form's Online Documentation link with user-defined documentation.

Widgets
  • In the Widgets to Display On Form field, select the widgets to add to the selected form. This field displays widgets from all products, allowing you to use widgets from one product on forms from a different products.
    For widgets, you do not need to specify a section on the form.

  • Click Submit.
