View A Provider Diagnosis

Details

Client diagnoses are display only for ProviderConnect administrators.

Steps

1. In the Main Menu click Lookup Client.

• In the Search Criteria screen enter the client's name, ID, social security number.
  Or, enter no data and click Search By Criteria to display all clients.

2. Click Search By Criteria.

• The Search Results screen displays.

3. Click a client ID number.

• The ProviderConnect Demographic screen displays for the client and agency.

4. In the Client Sidebar menu click Provider Diagnosis.

• The Provider Diagnosis screen displays.

5. Click the diagnosis date to view the client's provider diagnosis information.
