View A Provider Admission

View client admissions filed in ProviderConnect or Avatar PM.

• An admission must be filed in ProviderConnect by adding a new client in the Add New Client/Client Search link, or in the Admission form (Avatar PM).

1. In the Main Menu click Add New Client/Client Search.

• The Search Criteria screen displays.

2. Enter the client's last name and their social security number, in the format ###-##-####.

3. Click Search by Criteria.

• The Search Results screen displays.

4. Click the client ID number.

• The ProviderConnect Demographic screen displays for the client and agency.

5. In the Client Sidebar menu click Provider Admission.

• The Provider Admissions screen displays.

6. Click the client ID to view the client's provider admission information.
