Record problems in the client treatment plan

Record and track a client's problems associated with the diagnosis.


  1. Go to: Avatar CWS > Treatment Planning > Client Treatment Plan > Problems Section
  2. In the Select Library field, select the treatment plan library.
    Note: Problems can be selected from a library, or new problems can be entered.
  3. In the Select Problem To Edit field, choose the treatment plan problem.
    Note: If a problem has not been pulled from the treatment plan library, that problem can be selected. Problems that have not been saved display as "*NS" (not saved).
  4. In the Date Opened field, enter the problem opened date.
  5. In the Status field, select the problem status.
  6. If appropriate, enter the problem due date.
  7. If appropriate, enter the problem closed date.
  8. In the Staff Assigning field, enter the staff name of the person who identified the problem, and select Process Search.
  9. Select Ok.
  10. If appropriate, in the Staff Responsible field of the person responsible for tracking the problem, and select Process Search.
  11. Select Ok.
  12. The Predefined field displays Yes when the problem was selected from a library; it displays No for all other problems.
  13. Select Delete to remove the displayed treatment plan item.
