Record problems in the client treatment plan
Record and track a client's problems associated with the diagnosis.

Go to: Avatar CWS > Treatment Planning > Client Treatment Plan > Problems Section - In the Select Library field, select the treatment plan library.
Note: Problems can be selected from a library, or new problems can be entered. - In the Select Problem To Edit field, choose the treatment plan problem.
Note: If a problem has not been pulled from the treatment plan library, that problem can be selected. Problems that have not been saved display as "*NS" (not saved). - In the Date Opened field, enter the problem opened date.
- In the Status field, select the problem status.
- If appropriate, enter the problem due date.
- If appropriate, enter the problem closed date.
- In the Staff Assigning field, enter the staff name of the person who identified the problem, and select Process Search.
- Select Ok.
- If appropriate, in the Staff Responsible field of the person responsible for tracking the problem, and select Process Search.
- Select Ok.
- The Predefined field displays Yes when the problem was selected from a library; it displays No for all other problems.
- Select Delete to remove the displayed treatment plan item.
