Nursing Assessment - A. D. L. section

Record Activities of Daily Living information for the selected client.

  • This information includes tracking walking ability, bathing ability, elimination ability, as well as client risk factors and activities that are fundamental for self-care.

Prerequisite:

  1. In the Walking field, select the level of assistance needed.
  2. In the Dressing field, select the level of assistance needed.
  3. In the Bathing field, select the level of assistance needed.
  4. In the Oral Hygiene field, select level of assistance needed.
  5. In the Elimination field, select level of assistance needed.
  6. In the Check All That Apply field, select all activity of daily living aides from the check boxes. 
    Note: If "Use of Other" is selected, enter any associated comments in the Remarks field.
  7. In the Comments field, enter any associated comments.
  8. In the Elopement Risk field, select the appropriate value.
    Note: For this and the next 7 fields, if Yes is selected, enter any associated comments in the corresponding Remarks fields.
  9. In the Arson Risk field, select the appropriate value. 
  10. In the Excessive Water Consumption field, select the appropriate value.
  11. In the Sexual Behavior Risk field, select the appropriate value.
  12. In the Legal Difficulties field, select the appropriate value.
  13. In the Suicidal Risk field, select the appropriate value.
  14. In the Violence Risk field select the appropriate value. 
  15. In the Falls Risk field, select whether or not the client is at falls risk.
  16. In the Falls Risk (Selection) field, select the appropriate falls risk reason.
  17. In the Comments field, enter any associated comments to risk factors.
  18. In the Condition of Body field, select the appropriate condition of the client's body.
  19. In the Describe Body Part and Location field, enter any associated comments.
  20. In the Discharge Planning Needs field, select the appropriate needs following discharge.
  21. In the Comments field, enter any associated discharge planning comments.
  22. In the Client/Family Teaching Needs field, select the appropriate client/caregiver education.
  23. In the Comments field, enter any associated client/caregiver education comments.
