Nursing Assessment

Record a client's functioning using the Nursing Assessment form.

1. Go to: Avatar CWS > Assessments > Product Assessments > Nursing Assessment.
2. Select the correct Client and episode and click OK.
3. In the Assessing Date field, enter the assessment date.
4. In the Assessment Time field, in HH:MM AM, HH:MM PM, or HHMM in military time format or select Current Time.
5. In the Assessing Clinician field, search and select the assessing clinician's name, and matching results will dynamically display within the dropdown.
6. In the Height field, enter the client's height.
7. In the Weight field, enter the client's weight.
8. In the Blood Pressure field, enter the blood pressure in the ###/### format.
9. In the Position field, select the associated blood pressure position.
10. In the Pulse field, enter the client's pulse in the ## or ### format.
11. In the Position field, select the associated pulse position.
12. In the Respiration field, enter the client's respiration rate.
13. In the Temperature field, enter the client's temperature.
14. In the Eye Color field, type in the client's eye color. 
15. In the Hair Color field, type in the client's hair color.
16. In the Cultural/Language Needs field, type any cultural or language needs.
17. If applicable, complete the Medical History, Health Habits, ADL, and Summary Sections.
18. Once finished, click Submit.
