Immunizations Screen

Enter client immunization information.

Buttons

  • New Row - create a new row.
  • Delete Row - delete a row.
  • Save - submit changes made in the screen.
    Save is enabled when all required information is entered.
  • Close/Cancel - close the screen without saving changes.
  • Copy/Paste Row - select a row to copy or paste.
  • Copy Cell - select a cell to copy.
  • Paste Cell - select a cell to paste.

Fields

  • Vaccine - define the vaccine to be tracked by the agency.
  • Dose - select the number of times a vaccine requiring multiple doses will be administered.
  • Provided By - select the health maintenance provider.
    If Other is selected, several fields are disabled.
  • Date - enter the date the immunization was administered.
  • Refused - select whether the vaccine was refused.
    If Yes is selected, in the Reason field select the reason the vaccine was refused.
  • Time - enter the time the immunization was administered.
  • Amount - enter the dosage amount.
  • Unit - select the dosage unit.
  • Unit - Other - enter the unit if not listed.
  • Route - select the vaccine route.
  • Route - Other - enter the route if not listed.
  • Site - select the administration site.
  • Body Site - Other - enter the body site if not listed.
  • Manufacturer - select the manufacturer.
  • ​Lot # - enter the lot number.
  • Exp Date - enter the expiration date.
  • Ordered By - select the ordering practitioner.
  • Administered By - select the administering practitioner.
  • Administration Comments - enter administering comments.
  • Source of Immunization - select the information source.
    This applies when the extended dictionary data element in the Provided By field is defined.
  • Vaccine Statement Shown to Patient - select whether the vaccine statement has been shown to the client.
  • Date Vaccine Statement Presented to Patient - enter the date the vaccine statement was shown to the client.

    The following fields define information relating to the vaccine information statement (VIS) presented to the client.
  • Name of VIS Statement - enter the VIS statement name.
  • VIS Number - enter the VIS statement number.
  • VIS Edition Date - enter the VIS statement date.
  • VIS Edition Status - select the VIS statement status.
  • VIS Last Updated Date - enter the last date the VIS statement was updated.
