Define fields that will be required in the Diagnosis form.

1 - Axis II-1

2 - Axis III-1

3 - All Axis IV fields.

4 - Axis IV Remarks (If user responds 'Yes' to any Axis IV field, then the associated Axis IV Remark field becomes required.)

5 - Axis V Current GAF Rating.

Separate multiple values with '&'.
