Define which fields display in tables in the Benefit Plans (Insurance Plan Levels section), and Financial Eligibility forms (Billing Plan Assigned section).

Enter the following numbers. Separate multiple numbers with '&', with no spaces.

1 - Level Number

2 - Level Start Date

3 - Level End Date

4 - Deductible Type

5 - Deductible Amount

6 - Per Diem Rate

7 - Covered Charge Categories

8 - Practitioner Categories Necessary For Coverage
