Form Bundler

Create a group of forms that can be opened one at a time, or all at the same time.Form bundles display on the myAvatar menu and are accessed the same way as any other myAvatar form. These forms can be defined to be submitted in one application. Form Bundles can be removed using the Remove Form form.

Once a form bundle is created, users must be given access to the form bundle through the User Definition and User Role Definition forms.Forms can be opened at one time, or one after the other.As one form is filed, the next form in the bundle will open.

  • Rules for working with the Form Bundler.
  • Form types include:
    • Registration forms - create a new entity (client, staff, etc.), a new client episode, or client enrollment.  For example, Admission, User Definition, Practitioner Registration (Avatar PM).
    • Episode forms (Avatar PM only) - specify an episode type for submission. For example, Leaves, Add/Edit Partial Hospitalization Information form (Avatar PM).
    • Discharge forms - end a client episode or enrollment. For example, Discharge, Member Termination (Avatar MSO).

  1. Go to: RADplus Utilities > Menu Manager > Form Bundler
  2. In the Form Bundle Description field, type the form bundle description to display in the form bundle menu.
  3. In the Type of Bundle field, select:
    • Sequential - open each form in the bundle at a time. When one form in the bundle is submitted, the next form will open.
    • Simultaneous - open all forms in the bundle at the same time.
  4. In the Menu to Place Form Under field, select the myAvatar menu heading to place the form.
  5. The Current Order field displays the menu heading order.
  6. In the Order field, select the menu position.
  7. Menu positions are numbered. Selecting a different number changes the menu order.
  8. Click Print Current Menu to generate a report of myAvatar form menus.
  9. Follow the steps in the additional section below to complete the steps in the Included Forms section. 
  10. Click Submit.
