Product Update Form Description

Avatar eMAR is enhanced so that the "Give:" line within the 'Order Description' of certain pharmacy-type orders presents the 'Dose' both in terms of the dosage form and as an expression of strength. For example: "Give: 650 MG = 2 tablets every 6 hours, as needed" or "Give: 250 MG = 5 mL every 6 hours". This type of notation will be included when: 1) The 'Route of Administration' on the order is any of the following: "ORAL", "BUCCAL MUCOSA", "SUBLINGUAL", "GASTROSTOMY TUBE", "J-TUBE", or "TUBE FEED". 2) The 'Dosage Unit' ('Strength Unit') on the order does not contain either "%" or "-". ('Order Codes' for combination products list the strengths of the multiple active ingredients in the 'Strength Unit' field, separated by hyphens.) 3) The order was submitted from the Order Entry Console (Orders This Episode section) in "external pharmacy mode", or was not submitted in "external pharmacy mode" and the registry setting 'Use Pharmacy Fill Details For Order Description' is not set to "Y". (This type of notation will not be shown prior to the receipt of pharmacy fill details for a particular order, when pharmacy fill details are in use, because the pharmacy fill details can supply such a notation, and the pharmacy fill details can deviate from the original order, including the use of a different strength product.)

Scenario 1: eMAR - Exceed Defined Dosage - Interval Frequency = 'No', No scheduled 'Hours of Administration' - registry setting: 'Use Pharmacy Fill Details For Order Description' = "N"

Specific Setup:

The 'Avatar eMAR->General->Settings->->->Use Pharmacy Fill Details For Order Description' registry setting must be set to "N".
The 'Avatar eMAR->General->Settings->->->'Require client wristband scan in Avatar eMAR' registry setting must be set to "Y".
A client must have an active episode. (Client A).
“Client A” must have a ‘Date of Birth’, ‘Sex’ and address on file in the ‘Update Client Data’ form, as well as information filed in the ‘Allergies and Hypersensitivities’ form, ‘Diagnosis’ form, and in the ‘Height’ and ‘Weight’ fields in the ‘Vitals Entry’ form.
A frequency code must exist defined as: Interval Frequency = 'No', No scheduled 'Hours of Administration', and 'Times/24 Hours' = 2. (Frequency Code A).
An order must exist for Client A with Frequency Code A. (Order A).

Steps

Access Avatar eMAR for "Client A".
Select a cell under the current date for "Order A" and click [Administer].
Populate the required fields selecting a successful 'Administration Event' and file the administration.
Select a different cell under the current date for "Order A" and click [Administer].
Populate the required fields selecting a successful 'Administration Event' and file the administration.
Select another cell for "Order A" and click [Administer].
Click [Administer].
Populate the required fields selecting an unsuccessful 'Administration Event' and file the administration.
Select a different cell under the current date for "Order A" and click [Administer].
Populate the required fields selecting a successful 'Administration Event.
Click 'Warning' cell.
Validate 'Override Message' label is equal to "Administration of this dose will exceed the 'Times/24 Hours' defined for this order".
Set 'Override Text' field to "any value".
Click [OK].
File the administration.
Validate all cells administered contain the appropriate order data.

Scenario 2: Calculate Dose based on Quantity Ordered

Specific Setup:

The 'Avatar eMAR->General->Settings->->->Use Pharmacy Fill Details For Order Description' registry setting must be set to "Y".
Please log out of the application and log back in after completing the above configuration.
An order code for a liquid medication must exist. ( DULCOLAX LIQUID 1200 MG/15 ML )
A client must have an inpatient episode whose program or unit are configured in the ‘External Pharmacy Setup’ form. (Client A)
“Client A” must have a ‘Date of Birth’, ‘Sex’ and address on file in the ‘Update Client Data’ form, as well as information filed in the ‘Allergies and Hypersensitivities’ form, ‘Diagnosis’ form, and in the ‘Height’ and ‘Weight’ fields in the ‘Vitals Entry’ form.

Steps

Select “Client A” and access the Order Entry Console.
Search for and select "ADVIL 200 MG TABLET ORAL" in the 'new order' field.
Set the 'Dose' field to "3".
Select "Tablet" from the 'Dose Unit' field.
Select "AS NEEDED" from the 'Freq' field.
Select "ORAL" from the 'Route' field.
Set the 'Duration' field to "11" and click [Days].
Validate the 'Days Supply' field contains "11".
Validate the 'Dispense Qty' field contains "33".
Select "Tablet" from the 'Dispense Qty Unit' field.
Click [Add to Scratchpad].
Search for and select "DULCOLAX LIQUID (MAGNESIUM HYDROXIDE) 1200 MG/15 ML SUSPENSION ORAL" in the 'new order' field.
Set the 'Dose' contains to "30".
Select "Milliliter" from the 'Dose Unit' field.
Select "3 TIMES A DAY" from the 'Freq' field.
Set the 'Duration' field to "11" and click [Days].
Validate the 'Days Supply' field contains "11".
Validate the 'Dispense Qty' field contains "990".
Select "Gram" from the 'Dispense Qty Unit' field.
Validate the 'Directions' field contains "Take thirty (30) milliliters by mouth three times a day".
Click [Add to Scratchpad] and click Final Review].
Click [Save Override and Exit] and click [Sign].
Validate the 'order grid' contains "ADVIL 200 MG ORAL TABLETv1 Tablet, AS NEEDED" and ""DULCOLAX LIQUID (MAGNESIUM HYDROXIDE)"
Access the 'eMAR' widget.
Validate the 'Order Description Column' contains an order for "ADVIL 200 MG TABLET ORAL Give: 600 Milligram = 3 Tablet AS NEEDED".
Validate the 'Order Description Column' contains an order for "DULCOLAX LIQUID (MAGNESIUM HYDROXIDE) 1200 MG/15 ML ORAL SUSPENSION Give: 2400 Milligram = 30 Milliliter 3 TIMES A DAY".
Access the Order Entry Console.
Select the "DULCOLAX LIQUID" order and click [Modify].
Set the 'Dose' field to "1800".
Select "Milligram" from the 'Dose Unit' field.
Click [Add to Scratchpad].
Select the "ADVIL 200 MG ORAL TABLET" order and click [Modify].
Set the 'Dose' field to "800".
Select "Milligram" from the 'Dose Unit' field.
Click [Add to Scratchpad] and click Final Review].
Click [Save Override and Exit] and click [Sign].
Validate the 'order grid' contains "ADVIL 200 MG ORAL TABLET 1, AS NEEDED" and "DULCOLAX LIQUID (MAGNESIUM HYDROXIDE)".
Access the 'eMAR' widget.
Validate the 'Order Description Column' contains an order for "ADVIL 200 MG TABLET ORAL Give: 800 Milligram = 4 Tablet AS NEEDED".
Validate the 'Order Description Column' contains an order for "DULCOLAX LIQUID (MAGNESIUM HYDROXIDE) 1200 MG/15 ML ORAL SUSPENSION Give: 1800 Milligram = 22.5 Milliliter 3 TIMES A DAY".
