Product Update Form Description

The following changes are made for Florida FASAMS: 1) Support is added for allowing the Program Area selection to be manually overridden on the FASAMS Admission form. 2) Form logic is added to the FASAMS Placement form to prevent multiple placements from being entered for the same Program Area and Placement Type. 3) The 'Sub Contract', 'Placement Code', and 'Site Identifier' columns are added to the FASAMS Service configuration grid.

'FASAMS Placement' form - Prevent Conflicting Multiple Placements
An error message is received when attempting to add multiple placements in the 'FASAMS Placement' form for the same 'Program Area' and 'Placement Type'.
Value Added: The 'FASAMS Placement' form is enhanced to meet "Financial and Services Accountability Management System" (FASAMS) reporting requirements.

'Florida FASAMS Service Grid' - Additional Columns
The 'Florida FASAMS Service Grid' has been enhanced with the addition of the following columns: 'Sub Contract', 'Placement Code', and 'Site Identifier'. The 'STATEFORM.fl_fasams_service' SQL table has been updated accordingly. This gives users the option to define values in the columns for output to service related XML files created for "Financial and Services Accountability Management System" (FASAMS) reporting.

Value Added: 'Florida FASAMS Service Grid' enhanced to include three additional fields for use in "Financial and Services Accountability Management System" (FASAMS) reporting.

  • 'FASAMS Setup' form occurrence filed with at least one valid value in each of the following fields; 'Managing Entity(s)', 'FASAMS Program', 'FASAMS Guarantors', 'Financial Eligibility', 'Populate Client Demographics on Form Load', 'FASAMS Provider Sites'.
  • 'Admission' form filed for test child client for a program valid by 'FASAMS Setup' form values and aged under 18 as of the 'PreAdmit/Admission' date of the admission having no 'FASAMS Admission/Discharge' occurrences.
  • 'Admission' form filed for test adult client for a program valid by 'FASAMS Setup' form values and aged 18 or over as of the 'PreAdmit/Admission' date of the admission having no 'FASAMS Admission/Discharge' occurrences.
Steps
  1. Open the 'FASAMS Admission/Discharge' form for test client aged under 18.
  2. Set 'Effective Date' to "04/01/2022", click 'Program Area', verify "Child Mental Health" and "Child Substance Abuse" are the only values presented and select "Child Mental Health".
  3. Set 'Assessment Type' to "Admission", select values for 'FASAMS Programs' and 'Provider Site ID', set 'Admission Date' to "04/01/2022", complete all additional required fields and file the form.
  4. Open the 'FASAMS Admission/Discharge' form for test client aged under 18.
  5. Set 'Effective Date' to "04/01/2022", click 'Program Area', verify "Child Substance Abuse" is the only value presented and select it.
  6. Set 'Assessment Type' to "Admission", select values for 'FASAMS Programs' and 'Provider Site ID', set 'Admission Date' to "04/01/2022", complete all additional required fields and file the form.

  3. Open the 'FASAMS Placement' form for test client aged 18 or over and click [Add].
  4. Click 'Active Program Area(s) and select "Adult Mental Health", set 'Assessment Type' to "Open Placement", set 'Effective Date' to "04/01/2022", click 'Placement Type' and select "Inpatient Detoxification", set 'Start Date' to "04/01/2022" and file the form

Scenario 1: 'State Form Configuration Grid' form - Validate the 'Florida FASAMS Service Grid'
Specific Setup:
  • 'FASAMS Setup' form occurrence filed with at least one valid value in each of the following fields; 'Managing Entity(s)', 'FASAMS Program', 'FASAMS Guarantors', 'Financial Eligibility', 'Populate Client Demographics on Form Load', 'FASAMS Provider Sites'.
  • 'Dictionary Update' form filed with 'File' set to "State Form Table", 'Data Element' set to "(59001) Solution", and 'Dictionary Code' values "5 = Florida FASAMS Contract Grid" and "6 = Florida FASAMS Service Grid" active.
Steps
  1. Open 'State Form Configuration Grid' form.
  2. Select "Florida FASAMS Service Grid" from 'Solution' field and click [Export].
  3. Use the 'Save' window presented to save the file and exit the form.
  4. Make a copy of the file to another name for testing purposes and keep the original as a backup in the new file format that includes the three additional columns for use in making future changes.
  5. Open the file copy using software capable of reading/editing the file and verify the columns 'Sub Contract', 'Placement Code', and 'Site Identifier' exist.
  6. If there are no data values present in the file enter valid values for 'Avatar Program Code (Required)', 'Program Area (Required)', and 'Priority (Required) columns in the first row. Enter "12345" in the 'Sub Contract' column, "1" in the 'Placement Code' column, and "01" in the 'Site Identifier' column of the same row and save the file. If data already exists in the file enter "12345" in the 'Sub Contract' column, "1" in the 'Placement Code' column, and "01" in the 'Site Identifier' column of an existing data row and save the file.
  7. Open 'State Form Configuration Grid/ form.
  8. Select "Florida FASAMS Service Grid" from 'Solution' field and click [Import].
  9. Use the 'Open' window presented to select the file just exported and modified.
  10. Select "Yes" from the 'Proceed? This will overwrite any prior configuration and cannot be undone.' pop-up message presented.
  11. Select "OK" from the 'Information' pop-up message presented.
  12. Select "Florida FASAMS Service Grid" from 'Solution' field and click [Export].
  13. Use the 'Save' window presented to save the file and exit the form.
  14. Open the file using software capable of reading/editing the file and verify that the 'Sub Contract', 'Placement Code', and 'Site Identifier' columns are populated in the row previously updated.
  15. Using SQL query software verify the SQL table 'STATEFORM.fl_fasams_service' contains the columns 'sub_contract', 'placement_code' and 'site_identifier' that are populated for the row previously updated.
