Order Entry Console (Orders This Episode and Admission Med Reconciliation sections) - Sliding Scale Insulin orders
Note - These testing guidelines assume the user is skilled in the use of, at a minimum, the following:
- Order Entry Console - Admission Med Reconciliation
Scenario 1: Order Entry Console - Creating a new 'Sliding Scale Insulin' order
Specific Setup:
- An order code must have the 'Enable Sliding Scale Insulin' checkbox checked in the 'Order Code Setup' form. (Humalog)
- A 'Sliding Scale Insulin' template must exist.
- A client must have an active episode. (Client A)
- “Client A” must have a ‘Date of Birth’, ‘Sex’ and address on file in the ‘Update Client Data’ form, as well as information filed in the ‘Allergies and Hypersensitivities’ form, ‘Diagnosis’ form, and in the ‘Height’ and ‘Weight’ fields in the ‘Vitals Entry’ form.
Steps
- Select "Client A" and access the Order Entry Console.
- Hover over the various tabs and validate the text turns blue.
- Search for and select "HUMALOG 100 UNITS/1 ML SOLUTION INJECTION" in the 'New Order' field.
- Validate [Sliding Scale Instructions] exists.
- Validate the 'Dose' and 'Dose Unit' fields are required and have a red border around each field.
- Click [Sliding Scale Instructions].
- Select any value in the 'Template' field and click [Save].
- Validate there is no longer a red border around the 'Dose' and 'Dose Unit' fields and they are disabled.
- Set the 'Freq' field to any value.
- Validate the 'Dose' and 'Dose Unit' fields remain disabled and not required.
- Set the 'Duration' field to any value and click [Days].
- Validate the 'Addl Instructions' field contains the values from the template selected.
- Click [Add to Scratchpad] and [Sign].
- Validate the 'Order grid' contains an order for "HUMALOG 100 UNITS/1 ML SOLUTION INJECTION".
Scenario 2: OE NX - Sliding Scale Insulin order in Admission Reconciliation
Specific Setup:
- An order code must be configured for Sliding Scale Insulin via the 'Order Code Setup' form. (Humalog)
- A client must have an active episode. (Client A)
- “Client A” must have a ‘Date of Birth’, ‘Sex’ and address on file in the ‘Update Client Data’ form, as well as information filed in the ‘Allergies and Hypersensitivities form, and ‘Diagnosis’ form.
Steps
- Select “Client A” and access the Order Entry Console.
- Select the 'Admission Reconciliation' tab.
- Search for and select "HUMALOG 100 UNITS/1MLSOLUTION INJECTION" in the 'New Order' field.
- Click [Sliding Scale Instructions].
- Validate the 'Sliding Scale Insulin' dialog displays and click [New Row].
- Set the 'Blood Glucose From' field to "0".
- Set the 'Blood Glucose Up To' field to "69".
- Set the 'Units Before Breakfast' field to"2".
- Set the 'Units Before Lunch' field to "2".
- Set the 'Units Before Dinner' field to "2".
- Set the 'Units At Bedtime' field to "2".
- Click [New Row].
- Set the 2nd 'Blood Glucose From' field to "70".
- Set the 2nd 'Blood Glucose Up To' field to "100".
- Set the 2nd 'Units Before Breakfast' field to"1".
- Set the 2nd 'Units Before Lunch' field to "1".
- Set the 2nd 'Units Before Dinner' field to "1".
- Set the 2nd 'Units At Bedtime' field to "1".
- Click [Save].
- Validate the [+] is disabled.
- Validate the 'Dose' and 'Dose Unit' fields are not required and disabled.
- Select any value in the 'Freq' field.
- Validate the [+] is disabled.
- Validate the 'Dose' and 'Dose Unit' fields are not required and disabled.
- Set the 'Duration' field to "30" and click [Days].
- Validate the 'Addl Instructions' field contains "If blood glucose is 0-69: 2 Units before breakfast, 2 Units before lunch, 2 Units before dinner, 2 Units at bedtime, 70-100: 1 Unit before breakfast, 1 Unit before lunch, 1 Unit before dinner, 1 Unit at bedtime,".
- Click [Add to Scratchpad].
- Validate the 'Inpatient Medications Scratchpad' contains an order for "HUMALOG 100 UNITS/1 ML SOLUTION".
- Select the "HUMALOG 100 UNITS/1 ML SOLUTION EVERY DAY" order from 'Order Details'.
- Click [Sliding Scale Instructions].
- Validate the 'Sliding Scale Insulin Instructions' dialog is displayed and contains the previously entered values.
- Click [Close/Cancel].
- Validate the 'Addl Instructions' field contains "If blood glucose is 0-69: 2 Units before breakfast, 2 Units before lunch, 2 Units before dinner, 2 Units at bedtime, 70-100: 1 Unit before breakfast, 1 Unit before lunch, 1 Unit before dinner, 1 Unit at bedtime,".
- Click [Update Order] and [Reconcile & Sign].
- Validate that the 'Admission Med Reconciliation' tab is in view-only mode.

OE NX - Order Entry Console - Pre-Admit orders
Scenario 1: OE NX - Defer Activation of Pre-Admit episode - Validating before activation
Specific Setup:
- The registry setting 'Avatar Order Entry->Facility Defaults->Client Profile->->->Defer Activation of Orders in a Pre-Admit Episode' must be set to "Yes".
- The registry setting 'Avatar PM->Client Management->Movement Options->->->Allow Admission To File/Edit Pre-Admits' must be set to "Yes".
- The logged in user must be configured in the 'Order Entry User Definition' form to require validation and be able to validate orders.
- A client must have an active pre-admit episode. (Client A).
- “Client A” must have a ‘Date of Birth’, ‘Sex’ and address on file in the ‘Update Client Data’ form, as well as information filed in the ‘Allergies and Hypersensitivities’ form, ‘Diagnosis’ form, and in the ‘Height’ and ‘Weight’ fields in the ‘Vitals Entry’ form.
Steps
- Select "Client A" and access the Order Entry Console.
- Create orders for "ADVIL 200 MG TABLET ORAL" and "CHILDREN'S TYLENOL 160 MG TABLET, CHEWABLE ORAL".
- Access the 'Admission' form for "Client A".
- Select the previously filed row and click [Edit].
- Populate all required fields, admitting "Client A" into an inpatient episode.
- Select any other client from the 'My Clients' widget.
- Select "Client A" and access the Order Entry Console.
- Validate the 'New Order' field is disabled.
- Click [Activate] and validate the 'Start Date' for the orders are updated to the current date and the 'New Order' field is enabled.

- Create 3 orders: one for a Schedule II, one for a Schedule III, and one for a Schedule IV medication.
- Validate the 'Diagnosis' field is required for both the Schedule II and Schedule III medications.
- Validate the 'Diagnosis' field is not required for the Schedule IV medication.
Scenario 2: OE NX - Discharge Med Reconciliation - Diagnosis
Specific Setup:

- Avatar OE 2022 Update 88 and NX Release 2022.10.00.01 must be installed in order to utilize full functionality.
- The user logged into the application must have the 'Client Reported' checkbox checked in the 'Default to Client Reported in Home Medications' field in 'Order Entry User Definition'.
- Please log out of the application and log back in after completing the above configuration.
- A client must have an active inpatient episode. (Client A)
- “Client A” must have a ‘Date of Birth’, ‘Sex’ and address on file in the ‘Update Client Data’ form, as well as information filed in the ‘Allergies and Hypersensitivities form, and ‘Diagnosis’ form.
Steps
